Immunocompromised L monocytogenes, aerobic gram-negative bacilli, S pneumonia, N meningitis Vanc plus 3rd gen ceph and ampicillin
Neurosurgery, head trauma, cerebrospinal trauma Staphylococci, aerobic gram-negative bacilli, S pneumonia Vanc plus either 3rd gen ceph with anti-pseudomonal coverage or meropenem
